YPDSN has been facilitating Community Health Services cover ‘cradle-to-grave’ services that many of us take for granted. YPDSN provide a wide range of care, from supporting patients to manage long-term conditions, to treating those who are seriously ill with complex conditions. Most community healthcare takes place in people’s homes. YPDSN has conducting different types of community health services that you and your child may need include: financial support; multi-service agencies; communication resources; behavioural resources; sensory and motor services; social and recreation services; family support and respite services; school services; research opportunities.
Community health planning is a deliberate effort to involve the members of a geographically defined community in an open public process designed to improve the availability, accessibility, and quality of healthcare services in their community as a means toward improving its health status. Providing informal counseling, support and follow-up. Advocating for local health needs. Providing health services, such as monitoring blood pressure and providing first aid. Making home visits to chronically ill patients, pregnant women and nursing mothers, individuals at high risk of health problems and the elderly.
